Rules

You must fill in the form completely to qualify
Email addresses are optional and are not required to go forward with the form.
You will only qualify if you are from one of the locations listed in the City field of the form
The Hamper must be accepted as is (no substitutions)
We will not give the monetary equivalent of the hamper ($500) if the hamper is declined
If the winner should be disqualified or decline the hamper a new winner will be chosen and contacted
If you have won the hamper once you will not be able to win again within 1 calendar year
