Rules
- You must fill in the form completely to qualify
- Email addresses are optional and are not required to go forward with the form.
- You will only qualify if you are from one of the locations listed in the City field of the form
- The Hamper must be accepted as is (no substitutions)
- We will not give the monetary equivalent of the hamper ($500) if the hamper is declined
- If the winner should be disqualified or decline the hamper a new winner will be chosen and contacted
- If you have won the hamper once you will not be able to win again within 1 calendar year
- One Entry per household
